Thank you for your participation in PUENTE Learning Center’s 2024-2025 Healthcare Saturdays program!

We are excited to offer this scholarship opportunity to our graduating seniors as they prepare to pursue their undergraduate education.

Please read criteria and requirements carefully before filling out the form.

Scholarship Amount: $500

Who can apply:

• Must be a Healthcare Saturdays student currently in 12th grade and graduating in spring 2025.
• Must have attended 100% of our workshops throughout the program year (7 out of 7 workshops).
• Will be enrolled as a full-time undergraduate student during the 2025-2026 school year.

Documents you need before submitting application:

1. One letter of recommendation from someone who can speak highly of you. This can be a counselor, coach, supervisor, teacher, or other adult who knows you well (not family members).
2. Scholarship Prompt: Every future healthcare professional has a unique story of inspiration. Whether it’s a personal experience, a role model, or a meaningful moment, these experiences shape your passion for helping others and your commitment to making a difference. In a video no longer than three minutes or essay no longer than one page double spaced (Times New Roman, 12 Font), please answer the following: What experiences in your life have inspired you to pursue a career in healthcare?
3. Proof of college enrollment: documentation confirming your enrollment in an undergraduate program for the 2025-2026 academic year.

If you are selected as a scholarship winner, you are committing to:

*Participating in our virtual College Success Program – Avanzando Through College for the 2025-2026 school year. Avanzando Through College provides monthly workshops and one-on-one meetings to help you navigate your first year of college. As part of the program, you will have access to care packages, micro-scholarships, and additional resources.

-Scholarship winners will be announced in person during our Healthcare Saturdays Program Celebration on 4/26/25.

– Scholarship funds will be distributed August 2025 during the Avanzando Orientation.

Application Deadline: This application must be received by 8:00 AM Monday, April 7, 2025 to be considered for a scholarship award.
